Documents required for the issuance of an Identity Card

1. Identity document, if available (foreign citizen’s passport – if the person was a citizen of a state; Soviet-type passport (1974 model); temporary identity document that does not contain information regarding citizenship);
2. Decision on the recognition of stateless person status;
3. Civil status documents: birth certificate, marriage certificate and/or divorce certificate, certificate of change of surname and/or given name, birth certificates of minor children, death certificate of the spouse (if applicable), extract from the marriage record to confirm the registration or dissolution of marriage in the case of persons whose marriage was dissolved abroad and where the respective state does not provide for the issuance of a divorce certificate, notice regarding the cancellation, rectification and/or completion of a civil status record, as well as other civil status documents; extract from the marriage record (submitted by persons whose marriage was dissolved abroad and where the respective state does not provide for the issuance of a divorce certificate);
4. Documents attesting the right of ownership or the right to use residential premises, as provided by the Regulation on the Issuance of Identity Documents and the Registration of Residents of the Republic of Moldova:

• Declaration of accommodation; declaration by the owner or co-owners of the dwelling confirming that they are the legal holders of the dwelling. The declaration shall be submitted in writing either personally or through an authorized representative holding a notarized power of attorney or authorization certified by another person empowered by law, before the official authorized to register persons at their domicile or temporary residence, or it shall be notarized or certified by another person empowered by law;
• Notarized consent of the owner of the residential premises; proof of ownership of the dwelling (sale-purchase agreement, donation agreement, legal heir certificate, etc.);
• Title deed to the dwelling, concluded in accordance with the validity requirements established by law and registered with the cadastral authority;
• Allocation warrant for publicly owned housing subject to privatization under the law, issued by the local public administration authority (Mayor’s Office) after 29 November 2015, or, where applicable, reconfirmed by the local public administration authority (Mayor’s Office) if issued before 29 November 2015, or a supporting document issued by the local public administration authority (Mayor’s Office) establishing the right to use publicly owned housing subject to privatization;
• Tenancy agreement concluded in accordance with Chapter VIII of Law No. 75 of 30 April 2015 on Housing, between the dormitory resident and the lessor. In the case of students accommodated in dormitories under the administration of public educational institutions, a copy of the decision of the educational institution managing the dormitory and the order allocating the accommodation shall be submitted;
• Lease or sublease agreement concluded in accordance with Chapter VIII of Law No. 75 of 30 April 2015 on Housing;
• Leasing agreement concluded in accordance with the applicable legal provisions;
• Final court judgment recognizing the right to use the dwelling;
• Extract from the Register of Household Records of the respective locality confirming the registration of the dwelling and the registration of members of the farming household;
• Official request issued by the administration of a social assistance institution, specialized healthcare institution, family-type or residential placement service, penitentiary institution, military unit, educational institution, including those subordinated to the Ministry of Internal Affairs or the Ministry of Defence, placement centres, as well as other specialized institutions providing accommodation facilities, where the conclusion of a tenancy agreement is not mandatory for accommodating persons;

5.Proof of payment of the fees and charges established for the issuance of the identity document.
