Provisional residence for long-term medical treatment
Granting
List of documents required to grant provisional residence for long-term medical treatment:
Standard Application Form
A letter from the medical institution stating the diagnosis and the minimum necessary duration of treatment
Applicant's national passport (original and copy) with appropriate references made by the state border control authority (confirming the date and place of crossing the state border)
Confirmation from the medical/health care institution that the accommodation is provided for the requested period
Criminal record (in original) from the applicant's country of origin, legalized/apostilled by the authorities of the state of nationality. If the alien has been legally residing for at least two years in a country other than the state of his/her nationality, he/she shall present the criminal record or another document with the same legal value issued by the authorities of the country of residence, translated and legalized/apostilled, as well as the document confirming the right of residence, except for minors under 16 years of age.
Original and copy of the proof of health insurance valid for at least 3 months
Proof of means of subsistence and treatment in the amount corresponding to the category of residence applied for
A (one) 3x4 cm color photo.
Extension
List of documents required for extension of provisional residence for long-term medical treatment:
Standard Application Form
A letter from the medical institution stating the diagnosis and the minimum necessary duration of treatment
Alien's national passport (original and copy)
Alien's residence permit (original and copy)
Confirmation from the medical/health care institution that the accommodation is provided for the requested period;
Copy of health insurance obtained from the National Health Insurance Company
Proof of means of subsistence and treatment in the amount corresponding to the category of residence applied for
A (one) 3x4 cm color photo.
NOTES:
The documents must be submitted at least 30 days before the authorized end of foreigner’s stay in the Republic of Moldova.
The EU citizens must submit their documents at least 15 days before the authorized end of their stay in the Republic of Moldova.
Failing to meet the deadline for application is subject to administrative penalties.
