List of documents required to grant provisional residence for long-term medical treatment:<\/strong><\/h2>\n<\/div>\n<\/div>\n<\/div>\n
\n
\n
\n
    \n
  1. Standard Application Form;<\/span><\/li>\n
  2. The letter from the medical institution stating the diagnosis and the minimum duration required for the treatment (original and copy);<\/span><\/li>\n
  3. Applicant’s national passport (original and copy) with appropriate references made by the state border control authority (confirming the date and place of crossing the state border);<\/span><\/li>\n
  4. Confirmation from the medical\/healthcare institution that the accommodation is provided for the requested period (original and copy);<\/span><\/li>\n
  5. Criminal record (original and copy) from the applicant’s country of origin, legalized\/apostilled by the authorities of the state of nationality. If the foreigner has been legally residing for at least two years in a country other than the state of his\/her nationality, he\/she shall present the criminal record or another document with the same legal value issued by the authorities of the country of residence, translated and legalized\/apostilled, as well as the document confirming the right of residence, except for minors under 16 years of age;<\/span><\/li>\n
  6. Original and copy of the proof of health insurance valid for at least 3 months;<\/span><\/li>\n
  7. Proof of means of subsistence and treatment in the amount corresponding to the category of residence applied for <\/span>(the validity period of the bank statement must be no more than 10 days prior to its submission);<\/li>\n
  8.  A (one) 3×4 cm color photo.<\/span><\/li>\n<\/ol>\n<\/div>\n<\/div>\n<\/div>\n

    1. The documents must be submitted at least 30 days before the authorized end of foreigner\u2019s stay in the Republic of Moldova.<\/span><\/li>\n
    2. The EU citizens must submit their documents at least 15 days before the authorized end of their stay in the Republic of Moldova.<\/span><\/li>\n
    3. Failing to meet the deadline for application is subject to administrative penalties.<\/span><\/li>\n<\/ol>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n

      2. The letter from the medical institution stating the diagnosis and the minimum necessary duration of treatment (original and copy);<\/span><\/li>\n
      3. Applicant’s national passport (original and copy) <\/span>with appropriate references made by the state border control authority (confirming the date and place of crossing the state border);<\/span><\/li>\n
      4. Applicant’s residence permit (original and copy);<\/span><\/li>\n
      5. Confirmation from the medical\/health care institution that the accommodation is provided for the requested period (original and copy);<\/span><\/li>\n
